Flourish Wellness Membership

Membership includes a custom treatment plan including prescription refills, direct access to providers during business hours, in person visits, discounts on supplements and lab testing such as Dutch and ZRT, discount on name brand botox and Versatility products and services, free Inbody testing with macros & weight management recommendations. Price: $55 billed monthly

Initial Consulatation

Includes an extensive medical interview reviewing lifestyle and symptoms, a review of lab and treatment options. Lab costs are not included in this price.

Price: $125 due at time of service

Hormone Testing/Labs

Serum labs are billed through private insurance. Dutch testing priced per test, ZRT testing priced per test.

Price: Priced per test

In Person or Phone Follow Up

Review of lab results, propose treatment and/or supplement plan, short term prescriptions if needed. Cost of first follow up for members goes towards first month membership fee.

Price: $75 non-member/$55 member

Botox

Brand name botox, charged per unit.

Price: $12.50 per unit non-member/$11 per unit member
